When documenting residency of a member of the armed forces on active duty, item 26.c. on Form 4473 asks for "PCS Base/City and State." Is that asking for both the base as well as the city and state, or just one or the other?

You must document both the name of the military base and the city and state where the base is located. For example, “Fort Knox, Fort Knox, Kentucky.”
